We are a small, compassionate hospice organization seeking a detail-oriented and highly organized Medical Records Coordinator to join our team. This full-time, in-office role is responsible for ensuring the accuracy, completeness, and compliance of patient medical records in accordance with Medicare Conditions of Participation, state regulations, and agency policies.

Key Responsibilities
Maintain and organize electronic and/or paper medical records for hospice patients
Ensure documentation is complete, accurate, and compliant with Medicare and state hospice regulations
Monitor timeliness of documentation, including admissions, physician orders, certifications/recertifications, and IDT notes
Track and follow up on missing, unsigned, or incomplete documentation
Assist with internal audits and preparation for surveys or regulatory reviews
Process release of information requests in compliance with HIPAA guidelines
Prepare records for billing review and ensure documentation supports claims
Maintain secure storage and archiving of medical records in accordance with retention policies
Provide administrative support to the interdisciplinary team (nurses, social workers, chaplains, physicians) as needed
Maintain strict confidentiality of protected health information (PHI)
Qualifications
Required:
High school diploma or equivalent
Minimum 1β2 years of experience in medical records, health information, or healthcare administration
Working knowledge of HIPAA regulations
Strong attention to detail and organizational skills
Proficiency with electronic medical record (EMR) systems and Microsoft Office
Preferred:
Experience in hospice, home health, or long-term care
Familiarity with Medicare Conditions of Participation (CoPs)
Health Information Management certification (RHIT, RHIA, or similar)
