Responsibilities
- Provides clinical social evaluations and plans appropriate interventions based on evaluation findings.
- Identifies appropriate community resource referrals to address the client’s and the family’s practical and environmental needs.
- Acts as an advocate for both the client and the family to navigate the patient through the community system.
- Participates in the development of the total plan of care with the multidisciplinary team of clinicians, as appropriate.
- Develops and maintains contracts with public and private agencies as resources for the patient.
- Educates and instructs patients, family members, or other patient representatives, in community resources to promote the patient's health, safety, well-being and independent living.
Qualifications
REQUIRED QUALIFICATIONS
- Master of Social Work or a master’s degree in a related field required.
- Minimum one year experience in a health-related field required.
- Experience with database software or electronic medical record software required.
PREFERRED QUALIFICATIONS
- MSW and/or LCSW preferred. Hospice experience preferred.
