Position Summary
As a PRN Physical Therapist, you'll evaluate residents, develop individualized treatment plans, and provide evidence-based therapy designed to improve function, safety, and quality of life. You'll work closely with physicians, nursing staff, and rehabilitation professionals to help residents achieve their highest level of independence.
Key Responsibilities
Patient Evaluation & Treatment
Evaluate residents based on physician referrals.
Develop individualized treatment plans with measurable, functional goals.
Provide skilled physical therapy interventions to improve mobility, strength, balance, and overall function.
Reassess resident progress and modify treatment plans as appropriate.
Prepare discharge recommendations and educate residents and families on home exercise programs and safety strategies.
Collaboration & Communication
Partner with physicians, nurses, and the interdisciplinary care team to coordinate resident care.
Participate in care conferences and quality improvement initiatives.
Supervise Physical Therapist Assistants (PTAs), rehabilitation technicians, and students as appropriate.
Documentation & Compliance
Complete accurate, timely documentation in accordance with facility policies and regulatory requirements.
Ensure documentation supports appropriate billing and reimbursement.
Maintain knowledge of Medicare, insurance guidelines, and current rehabilitation best practices.
Department Support
Help maintain therapy equipment and ensure treatment areas are safe, clean, and organized.
Participate in staff education, in-services, and departmental meetings.
Provide coverage at nearby affiliated facilities as needed based on resident care needs.
Qualifications
Graduate of an accredited Physical Therapy program.
Current or eligible North Carolina Physical Therapy license.
Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.
Ability to safely assist residents with transfers and mobility, including lifting up to 50 pounds when necessary.
Excellent communication, organizational, and clinical decision-making skills.
Ability to work independently while contributing to a collaborative therapy team.
