The Program Support Assistant (Patient Centered Management Module Coordinator) is a GS-07 role located at the Bay Pines VA Healthcare System within the Primary Care Medicine Service. The primary focus of this position is the management and oversight of the Patient Centered Management Module (PCMM) software program. This system is critical for monitoring primary care services and feeds into a national database used to track performance measures, wait lists, and labor mapping.

Qualifications:

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 05/04/2026.

Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-06. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.

You may qualify based on your experience as described below:
  • Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-06 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:
  • Database Management: Managing and maintaining the integrity of complex healthcare software and national databases.
  • System Troubleshooting: Serving as a technical expert to identify, investigate, and resolve software errors or data inconsistencies.
  • Data Analysis: Collecting and analyzing information to create detailed reports and templates for management.
  • Staff Training: Providing orientation, guidance, and ongoing technical advice to clinical and administrative staff.
  • Process Improvement: Studying work methods and resource utilization to recommend ways to improve organizational efficiency.
  • Administrative Coordination: Managing workload assignments, monitoring provider capacities, and ensuring accurate patient enrollment.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment.

Physical Requirements: Work is sometimes sedentary but requires walking, standing and carrying of medium weight items, such as patient records and books. Incumbent must have an even temperament and behave in a professional manner.

Responsibilities:

Major Duties
- Program Support Assistant: Responsible for establishing and managing PCMM team and position set up for all Primary Care & Specialty Teams within Primary Care Service.
- Data Analysis and Reporting: Collects and analyzes complex data to prepare reports regarding panel sizes and workload patterns for the Chief of Primary Care. Assist with encounter reports, utilization reports, and to include but not limited to profile management, submitting a LEAF for new clinic set up, cancellations, modifications, and inactivation's for all clinics within Primary Care Service.
- Database Maintenance: Assigns patients to PC teams, discharges patients from the database when disenrolled, maintains accurate enrollment data.
- Patient Coordination: Enrolls patients into primary care teams, assigns them to appropriate providers, and manages large-scale patient reassignments when necessary.
-Operational Improvement: Efficiently performing a variety of administrative tasks, streamlining clerical processes, and identifying opportunities to enhance workflow and office productivity.
- Administrative Support: Assists with the Manages PACT (Patient Aligned Care Team) staffing and room assignments while ensuring compliance with VHA regulations and healthcare policies. Provides comprehensive administrative support in the Primary Care office by managing daily office functions, coordinating communications, and implementing process improvements to enhance workflow efficiency and overall operational effectiveness.
