ESSENTIAL JOB FUNCTIONS:
Performs tasks pertinent to age of patient (infancy to geriatric).
At the written order from a CVHS Provider, makes outside referral appointments.
Serves as a liaison between providers and patients for outside referrals.
Maintains a Referral Data Base.
Prints out a referral list each day.
Follows a tracking system by following up on all referrals. Provides written documentation of appointments kept or not kept.
Receives written or verbal documentation from providers for missed appointments.
Provides assistance with Insurance forms as needed prior to making appointments.
Faxes completed referral forms to appropriate facility.
Coordinates with the provider and pharmacy, appointments requiring a special preparation.
Provides special instruction sheets as needed.
Supports a team based approach to patient-centered medical care.
Attends work as scheduled.
Performs other duties as assigned.

Qualifications:
EDUCATION, TRAINING AND EXPERIENCE:
Medical terminology a plus.
