CVHS is seeking a Clinical Pharmacy Services Representative to serve as a point of contact for both patients and clinic staff to optimize access to the clinical services available at CVHS. The Clinical Pharmacy Services Representative will focus on assisting pharmacists, and other clinic staff, in managing the medication therapy of patients which could include prescription prior authorizations, specialty pharmacy assessments, conducting medication history reviews or scheduling Medicare Annual Wellness visits. Their responsibilities extend to collaborating with other healthcare professionals to optimize patient outcomes, making them an essential link between patients and their medication or clinical pharmacy needs. CVHS is looking for someone who has a high school diploma or equivalent, has great interpersonal skills both in person and over the phone, has the capability to use common computer programs including spreadsheets, word processing documents, and electronic medical records, has good organizational skills and the ability to schedule and prioritize tasks, and is proficient with common office resources (phones, computers, printers, fax machines, and scanners).

Qualifications:EDUCATION, TRAINING AND EXPERIENCE:
High School Diploma or equivalent.
Pharmacy Technician License (preferred, not required)
On-the-job training and experience under the supervision of a licensed pharmacist.
