Your Day-to-Day Life at Custom:
As a Physical Therapist at Custom Home Health, you will:
- Deliver individualized care that empowers patients to achieve their health and independence goals.
- Collaborate with RNs and interdisciplinary teams to design and implement personalized care plans.
- Educate patients and their families on therapy techniques, exercises, and treatments to promote lasting well-being.
- Coordinate and administer treatments that enhance physical function and overall health.

What We’re Looking For:
- Licensed Physical Therapist: Must hold a valid Physical Therapy license.
- 1 Years of Physical Therapy Experience: Prior experience preferred, but exceptional new graduates are encouraged to apply.
