Responsibilities- Provide direct nursing care to residents in accordance with physician orders, individualized care plans, and nursing standards.
- Administer medications, treatments, and therapies safely and accurately.
- Assess residents, monitor for changes in condition, and promptly notify the appropriate healthcare provider.
- Accurately document resident care, medication administration, and clinical observations in the electronic health record.
- Supervise and delegate nursing tasks to Certified Nursing Assistants (CNAs) while providing guidance and support.
- Assist with admissions, discharges, transfers, and resident assessments.
- Communicate effectively with residents, families, physicians, and the interdisciplinary care team.
- Perform nursing procedures within the scope of practice, including wound care, catheter care, and other treatments as assigned.
- Ensure physician orders are implemented accurately and completed in a timely manner.
- Maintain infection prevention and control practices while promoting a safe environment for residents and staff.
- Participate in quality improvement initiatives, staff education, and survey readiness activities.
- Comply with all federal, state, local, and facility policies, including resident rights and HIPAA regulations.
- Promote a culture of compassion, professionalism, teamwork, and resident-centered care.
- Perform other duties as assigned to support the nursing department and ensure quality resident care.
- Valid and current Licensed Practical Nurse (LPN) license.
- Completion of an accredited practical nursing program.
- Prior experience in a clinical or healthcare setting is preferred.
- Strong knowledge of nursing care practices, medication administration, and patient safety protocols.
- Good communication and interpersonal skills to interact effectively with patients, families, and healthcare staff.
- Ability to work under supervision and as part of a multidisciplinary team.
