Key Responsibilities
- Conduct patient evaluations and develop treatment plans
- Manage chronic conditions including CKD and hypertension
- Assist with dialysis patient care and follow-up
- Review and interpret lab results
- Educate patients on kidney health and treatment plans
- Coordinate care with physicians and healthcare team
- Ensure accurate documentation in EMR
Qualifications
- Master’s Degree in Nursing (Family Nurse Practitioner)
- Active Tennessee NP license (or ability to obtain)
- Nephrology, internal medicine, or primary care experience preferred
- Experience with chronic disease management
- Strong communication and clinical decision-making skills
Compensation & Benefits
- Salary: $110,000 – $130,000 per year (based on experience)
