Job Title: Healthcare Consultant I - FLA - (Hialeah) (Bilingual – English/Spanish)
Location: Miami-Dade County, FL (Must reside in or near Hialeah: 33010, 33012, 33014, 33015 & surrounding areas)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (Standard Business Hours)
Work Setting: Work From Home Field Visits (50–75% Travel)
Pay rate: $36/hr
About the Role
We are seeking a self‑motivated, energetic, and detail‑oriented Case Management Coordinator to join our Care Management team. This role combines remote work with frequent in‑person member visits across Miami‑Dade County. The ideal candidate is highly organized, tech‑savvy, and comfortable managing a caseload that includes both supportive and medically complex members.
This is not a nursing role. Candidates should not hold RN/LPN clinical responsibilities but must have care coordination, behavioral health, social work, or long‑term care experience.Mileage reimbursement is provided for travel.

Key ResponsibilitiesCare Coordination & Member Support
- Conduct comprehensive member assessments telephonically and/or face‑to‑face in homes, Assisted Living Facilities (ALFs), and Skilled Nursing Facilities (SNFs).
- Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.
- Create, implement, and monitor individualized care plans to address member health needs.
- Identify and coordinate services such as prior authorizations, PCP and specialist coordination, medication reviews, condition management, and community resources.
Member Engagement & Advocacy
- Utilize motivational interviewing and influencing skills to promote member engagement and healthy lifestyle changes.
- Provide coaching, education, and resources that empower members to make informed health decisions.
- Participate in multidisciplinary case reviews to ensure optimal outcomes.
- Identify and escalate any quality-of-care concerns as required.
Monitoring, Documentation & Compliance
- Track progress toward care plan goals and adjust plans as needed.
- Maintain accurate documentation in compliance with company policies, regulatory standards, and accreditation requirements.
- Use case management tools, data, and quality processes to support decision‑making and service coordination.
Required Qualifications
- Bilingual (English & Spanish) — fluent speaking, reading, and writing.
- Miami-Dade County residency (Hialeah or nearby required).
- Ability to travel 50–75% of the time for face‑to‑face member visits.
- Minimum 1 year of experience in behavioral health, long‑term care, care coordination, or a related setting.
- Previous case management experience.
- Strong verbal and written communication skills.
- Ability to multitask and adapt to a fast‑paced environment.
Preferred Qualifications
- Experience working in Managed Care.
- Proficiency in Microsoft Word, Excel, and Outlook.
- Experience with discharge planning or working in healthcare/social service environments.
Education
- Bachelor’s Degree required — preferably in Social Work, Human Services, Psychology, or a related field.
