Pharmacist (RPh)

Crystal Medical Supplies is looking to make an immediate hire for a Pharmacist on-site in Los Angeles, CA. We are dedicated to providing exceptional patient care and innovative pharmacy services. We are seeking a highly motivated and experienced Pharmacist to join the team. This is an excellent opportunity to utilize your skills and make a significant impact on patient outcomes. This is a full-time, on-site position and you must be licensed in California.

Overview

The Pharmacist will function as an integral part of our interdisciplinary healthcare team, providing comprehensive services to patients in Cash Pay Prescriptions. This role involves patient interaction, collaborating with physicians and other healthcare professionals, and contributing to the development and implementation of innovative pharmacy programs.

Responsibilities

• Provide comprehensive medication management (CMM) services, including medication reconciliation, medication therapy optimization, and patient education.
• Collaborate with physicians and other healthcare providers to develop and implement individualized patient care plans.
• Conduct patient interviews, assess medication adherence, and identify drug-related problems.
• Order and interpret laboratory tests related to medication management (if authorized).
• Provide patient counseling and education on medication use, potential side effects, and adherence strategies.
• Participate in quality improvement initiatives and contribute to the development of clinical protocols and guidelines.
• Stay up-to-date on current pharmacotherapy guidelines and best practices.
• Participate in interdisciplinary team meetings and contribute to collaborative care.

Qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
• Current and active pharmacist license in the State of California.
• Board certification in a relevant specialty is highly desirable.
• Knowledge of pharmacotherapy, pharmacokinetics, and pharmacodynamics.
• Excellent communication, interpersonal, and patient counseling skills.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of California pharmacy law and regulations.
• NPI number.
