Specialty Clinic Case Manager (RN)

Crusader Community Health is looking for a compassionate, detail-driven Specialty Clinic Case Manager to join our dedicated team. In this role, you’ll ensure patients receive timely access to specialty medications and coordinated care in clinics such as Hepatitis C (HCV) and PrEP. What You’ll Do
As the Specialty Clinic Case Manager, you will play a key role in supporting our specialty clinics by:
Managing prior authorization requests and staying up to date on formulary changes
Navigating patient assistance and copay grant programs
Building and maintaining strong relationships with specialty pharmacies
Monitoring and managing patient follow-up for labs, imaging, referrals, and specialist consults
Supporting clinic workflows and helping implement process improvements
Scheduling patients into specialty clinics and maintaining clinic rosters
Providing training and education to medical support staff
Partnering with providers during specialty clinic sessions
Participating in clinic meetings and staying current on specialty clinic guidelines
This role is ideal for a highly organized RN who enjoys both patient interaction and behind-the-scenes care coordination.
What We’re Looking For
Education & Credentials:
Graduate of an approved school of Registered Professional Nursing
Current Illinois RN license
CPR certification
Meets Continuing Education requirements
Experience:
Previous RN experience required
Ambulatory care experience preferred
Knowledge & Skills:
Understanding of prior authorization processes, managed care formularies, and patient assistance programs
Experience with Electronic Medical Records (eClinicalWorks preferred)
Strong communication skills—written and verbal
Ability to work independently, prioritize tasks, and handle changing demands
Professionalism, attention to detail, and commitment to quality
Ability to travel between locations as needed
