NOW HIRING – LPN CARE PLAN NURSE!
We are currently accepting applications for a Licensed Practical Nurse (LPN) – Care Plan Nurse to join our team within our long-term care and rehabilitation facility specializing in geriatric care.
This position is responsible for assisting with the development, implementation, review, and updating of individualized resident care plans to ensure the highest quality of care is provided in accordance with each resident’s needs.
We are seeking a dependable, organized, and compassionate LPN who demonstrates strong communication skills, attention to detail, and the ability to work collaboratively with nursing staff and other departments. The ideal candidate is committed to maintaining accurate documentation, supporting regulatory compliance, and ensuring our residents receive quality, resident-centered care.

Qualifications
- Current Alabama LPN License or Temporary LPN License
- Long-term care nursing experience preferred
- Previous care planning experience preferred, but training may be available
- Knowledge of resident assessments, care plans, and long-term care regulations preferred
- Strong organizational, documentation, and communication skills
- Ability to work independently and collaboratively with an interdisciplinary team
- Must be at least 18 years of age
- Must pass background check and drug screen
- Positive attitude toward the elderly and a commitment to providing quality resident care
Essential Job Functions
Responsibilities include, but are not limited to:
- Assist in developing, implementing, reviewing, and updating individualized resident care plans.
- Participate in interdisciplinary care plan meetings and resident care conferences.
- Work closely with nursing staff, department managers, residents, and families to ensure care plans accurately reflect resident needs and preferences.
- Review resident assessments, physician orders, and clinical documentation to identify changes requiring care plan updates.
- Ensure resident care plans are accurate, complete, current, and consistent with facility policies and applicable regulations.
- Assist with coordinating care plan meetings and communicating with residents and responsible parties.
- Monitor care plan implementation and collaborate with nursing staff to address identified needs.
- Maintain accurate and timely electronic documentation.
- Assist with quality assurance and performance improvement initiatives related to resident care planning.
- Support the interdisciplinary team in promoting resident safety, dignity, and quality of life.
- Perform additional nursing and administrative duties as assigned within the scope of LPN practice.
