Responsibilities
- Assess residents’ psychosocial needs and develop appropriate care plans
- Provide counseling and support to residents and families
- Coordinate discharge planning and community resource referrals
- Advocate for residents’ rights and well-being
- Collaborate with nursing staff, administration, and interdisciplinary teams
- Assist with care conferences and family communication
- Maintain accurate documentation and compliance with regulatory requirements
- Licensed Social Worker (LSW) preferred but not required
