Position Summary:
The MDS Coordinator is responsible for coordinating and overseeing the completion of the Minimum Data Set (MDS) assessments, ensuring compliance with federal and state regulations, and supporting accurate reimbursement processes. This role plays a vital part in ensuring quality care outcomes and maintaining our high standards of resident care.
Responsibilities:
- Coordinate and complete MDS assessments in accordance with all state and federal guidelines.
- Ensure accuracy, timeliness, and compliance of all documentation related to resident assessments.
- Collaborate with the interdisciplinary team (IDT) to develop individualized resident care plans.
- Monitor and evaluate resident care needs and ensure documentation reflects clinical services provided.
- Educate and support staff regarding MDS processes, care planning, and regulatory requirements.
- Participate in quality improvement initiatives, audits, and facility compliance programs.
- Maintain knowledge of current reimbursement systems and regulatory updates affecting MDS and care planning.
- Current Registered Nurse (RN) license in the state of Ohio (required).
- Prior experience as an MDS Coordinator or in a similar role in long-term care (preferred).
- Strong knowledge of MDS 3.0, RAI process, and CMS regulations.
- Excellent organizational, assessment, and communication skills.
- Ability to work collaboratively with interdisciplinary teams.
- Strong attention to detail and accuracy in documentation.
