Essential Duties & Responsibilities
- Develop and maintain referral relationships within assigned territory
- Serve as the primary liaison between healthcare providers, referral sources, patients, and families
- Educate referral sources on services, programs, and admission criteria
- Conduct routine visits to hospitals, physician offices, assisted living facilities, and community partners
- Collaborate with clinical and admissions teams to ensure smooth patient transitions
- Identify opportunities for business growth and community outreach
- Participate in networking events, health fairs, and marketing initiatives
- Maintain accurate records of referral activity and territory development efforts
- Monitor market trends and competitor activity within assigned territory
- Active and unrestricted LPN or RN license in the State of Ohio
- Previous experience in healthcare marketing, admissions, case management, or business development preferred
- Strong understanding of healthcare systems and referral processes
- Excellent communication, relationship-building, and presentation skills
- Ability to work independently and manage territory effectively
- Proficient with Microsoft Office and CRM/documentation systems
- Valid driver’s license and reliable transportation required
- Experience working in skilled nursing, rehabilitation, home health, hospice, or long-term care
- Established relationships within the Versailles and/or Fairborn healthcare communities
- Strong organizational and problem-solving abilities
