Summary

The Medication Monitoring Assistant position provides medication monitoring for agency clients and coordinates with the pharmacy to ensure that medications are always up to date and filled in a timely manner.

Essential Functions

  • Provides medication monitoring services to CrossWinds clients by traveling to client residences each day, twice a day for some clients.
  • Coordinates services between pharmacies, Medication Clinic, and nursing staff.
  • Meets with clients weekly to refill locked medication boxes, coordinate with providers, and educate on medications where appropriate.
  • Responsible for communicating questions or concerns to supervisor regarding medication monitoring provided to clients.
  • Coordinates with the client’s pharmacy to ensure that medications are filled and picked up in a timely manner.
  • Ensures that the client’s treatment team is aware of any client changes, such as the client going out of town, or a change in the client’s phone number and/or address.
  • Notifies supervisor directly with any schedule changes for clients and will report no shows for medication monitoring.
  • Responsible for completing documentation of medication monitoring notes for clients.
  • Submits timecards and all work-related paperwork in a timely manner and in an acceptable format as determined by agency requirements.
  • Observes and actively supports CrossWinds policies.
  • Establishes and maintains a positive working relationship and professional demeanor with clients and families, external community partners, CrossWinds staff members, and visitors.

Work Environment

This job operates in the community, client homes, and a professional office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.

The employee must occasionally lift or move up to 25 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Language skills are required, including the ability to read, write, comprehend, and speak the English language. This position also requires the ability to interpret written materials, write reports and business correspondence, and effectively present and respond to questions.

Requirements

Skills & Abilities

Education & Experience

High school diploma or equivalent is required. Associate’s Degree and prior experience in the health and human services field is preferred.

Certifications & Licenses

Certification as a Certified Medication Assistant (CMA) is required.

Computer Skills

Computer experience in Microsoft Office programs and basic computer fluency.

Additional Qualifications

Ability to work with persons with serious emotional disturbance and/or severe and persistent mental illness; possess demonstrated interpersonal skills and ability to react effectively in a variety of human service situations; ability to prioritize assignments; ability to work independently and as a member of a therapeutic team.
