What are the responsibilities and job description for the Intake Coordinator position at Crossroads Health?
Crossroads Health/New Directions is a 501(c)(3) non-profit organization with facilities located in Mentor, Painesville, and Pepper Pike serves Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, extended housing and recovery services—no matter one's ability to pay. Our mission is to provide hope, healing, and healthcare to everyone.
We are currently looking to add a Intake Coordinator to our New Directions Team. The Intake Coordinator will assist clients in successful recovery by providing highly effective intake process and by effectively managing payer requirements in a manner that supports "seamless" and "client friendly" continuity of care. Ensure prompt and thorough admissions scheduling, planning and management.
Most importantly, the position will:
- Provide trauma informed services to clients, families and referents primarily in the office and by telephone.
- Receive intake assessment/referral/program information calls and complete all required clinical and financial paperwork including but not limited to entering information into agency software.
- Provide information to families, clinical personnel and/or referral source regarding available services and financial details.
- With the Assistant Clinical Director, manage schedule of Diagnostic Assessments and Admissions.
- Ensure prompt scheduling of admissions and assist in admission process.
Position Requirement:
- High School Diploma required, Associates/Bachelor's degree preferred.
- One year of experience in a community mental health, social service, or medical office setting, with substantial patient/client interaction.
- Demonstrated experience in screening and intake. (preferred)
- CDCA Licensed in the State of Ohio.
