Crossroads Family Care is seeking a compassionate and dedicated **Family Nurse Practitioner (FNP)** to join our primary care team in East Texas. As a Federally Qualified Health Center (FQHC), Crossroads is committed to expanding access to high-quality, affordable healthcare and improving the health of the communities we serve.
The Family Nurse Practitioner will provide comprehensive primary and preventive care to patients across the lifespan. What You'll Do
- Provide comprehensive primary care to patients across the lifespan
- Perform examinations, assessments, diagnoses, and treatment planning
- Manage acute and chronic health conditions
- Order and interpret appropriate diagnostic tests and screenings
- Prescribe medications and treatments within scope of practice
- Promote preventive care, wellness, and patient education
- Collaborate with medical, dental, behavioral health, and other members of the care team
- Maintain timely and accurate clinical documentation
- Participate in quality improvement initiatives that support positive patient outcomes
Qualifications
- Master's or doctoral degree in nursing from an accredited program
- Current Texas APRN licensure or eligibility for Texas licensure
- Current national certification as a Family Nurse Practitioner
- Prescriptive authority in accordance with Texas requirements
- Current BLS certification
- Strong clinical judgment, communication, and interpersonal skills
- Commitment to providing compassionate, patient-centered care to diverse populations
