What are the responsibilities and job description for the Care Coordinator position at Crosspoint Human Services?
Our Care Coordinators are a vital part of our mental health services team. They assist our Person Served navigating life in the community and help them to set personal goals. If you have a passion for helping others and teaching them skills for independence to improve the quality of their life this position is for you. Our Care Coordinators are Case Managers that lead group activities and teach our Person Served how to access services within the community and they also assist with career choices and job services.
Job Responsibilities:
Duties include, but are not limited to:
- Assess individual's history, mental status, strengths, needs, abilities, preferences and resources including recommendations for treatment.
- Provide skills training to promote independence, develop personal support systems, and/or housing readiness.
- Provide assistance and ongoing support for obtaining/maintaining a safe, stable living environment.
- Provide or arrange for employment and vocational support and skills training.
- Provide or assist in arranging for needed support services.
Job Requirements:
- Requires a Bachelor's degree in counseling or related field.
- Knowledge of serious mental illness, addictions and developmental disabilities.
- Excellent assessment, counseling and training skills.
- Valid driver's license and proof of automobile insurance.
