What are the responsibilities and job description for the Childrens Targeted Case Manager position at Cross Roads Case Management, LLC?
1 position will cover the greater Augusta/Waterville area and 1 position will cover the greater Skowhegan area. Wednesdays required office day in Pittsfield.
Children’s Targeted Case Managers (CTCM) will work with youth birth to 21 who are struggling with mental health and or developmental disabilities. Children’s Targeted Case Managers will collaborate with clients, guardians service providers and other agencies to ensure client needs are being met. Children’s Targeted Case Managers meet with clients and guardians to provide support in establishing and reaching goals.
Responsibilities:
· Demonstrate strong collaboration with service providers involved with client care through regular contact around provision of services
· Ensure services are provided to clients in a manner consistent with trauma-informed practices, emphasizing strengths, privacy, individuality, choice, and cultural differences
· Provide services that empower client and family system to utilize and build their strengths, resources, and supports to meet goals as independently as possible
· Provides services in an ethical manner by maintaining professional relationships with the client and in working within the role as case manager
· Maintain and complete accurate client files according to agency policies, procedures and in compliance with applicable law, licensing, contracting, MaineCare regulations, consent decree and the rights of recipients
· Fully participate in required supervisions and trainings by coming prepared, actively engaging and documenting as required
· Independently and effectively manage caseload, including scheduling of client appointments, meetings and office time for completion of required paperwork
· Bill 25 hours per week
Work Requirements:
· Assessing needs and completing a comprehensive assessment
· Developing an individualized care plan and crisis plan
· Complete a CANS assessment every 90 days
· Make and monitor referral for services
· Coordinate services
· Ongoing monitoring and documentation of safety, well-being, and stability, as well as monitoring the progress toward the goals identified in their individual care plan
Qualifications:
· To be considered for this position applicants must have a Bachelor’s degree in Psychology, Mental Health and Human Services, social work, nursing, social and behavioral sciences, sociology, education or related field. Case managers must attend 20 hours of training yearly.
· Ability to work in client’s home and in the community.
· Strong English written and verbal communication skills are required.
· Reliable transportation and vehicle insurance
· At least 2 years case management experience is preferred
· Knowledge of Section 13 of the MaineCare Benefits Manual is preferred
· Working knowledge of Microsoft Office, Microsoft Outlook and Zoom and other video conferencing software
Working Conditions:
· Targeted Case Managers will work in client’s homes and in the community in Somerset, Penobscot, Piscataquis, and surrounding counties.
· A minimum of once weekly travel to the Pittsfield office.
· Must be able to work in homes with pets and in homes where there is cigarette smoking when case managers are not present.
