Key Responsibilities

• Evaluate and treat patients recovering from injuries, surgeries, or chronic conditions in a home care environment.
• Design individualized treatment plans that integrate exercises, hands-on therapy, and assistive devices to alleviate pain and enhance mobility.
• Document treatment sessions meticulously, tracking patient responses, and measuring progress against established therapeutic goals.
• Regularly reevaluate patient conditions, adjusting treatment plans as necessary to meet evolving therapeutic needs.
• Recommend appropriate assistive devices to support patients’ self-care and mobility.
• Educate patients, caregivers, and family members about therapy techniques, assistive devices, and home exercise programs.
• Maintain current knowledge of physical therapy practices through participation in workshops, professional organizations, and continuous education opportunities.

Patient Population / Client Focus

Our Physical Therapists work with a wide-ranging patient demographic, including individuals recovering from orthopedic surgeries, neurological conditions, and other physical impairments. This role requires adept communication and interpersonal skills to effectively connect with patients and their families, ensuring they feel supported and understood in their recovery journeys.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid state license to practice physical therapy is mandatory.
• Minimum of two years of relevant professional experience in physical therapy is required.
• Strong written and verbal communication skills to interact effectively with patients, families, and other health professionals.
• Proficient in word processing and basic computer applications.
• High flexibility and adaptability to provide care in various home settings.

Preferred Experience

• Prior experience in home health settings is preferred but not mandatory.
• Experience with rehabilitation equipment and assistive technology is advantageous.

Compensation and Benefits
