Job Summary
Long Term Care Registered Nurse (RN) [Location] This position involves providing holistic, age-specific care to residents within a long-term care facility. The RN will utilize the nursing process to assess, diagnose, plan, implement, and evaluate resident care, ensuring high-quality standards are maintained. The role emphasizes resident advocacy, team collaboration, and compliance with legal and professional standards.

Job Details

  • Position Description:
    The RN utilizes an interpersonal process that uniquely focuses on the total individual and their role in family, culture, and society. The RN employs the nursing process, including assessment, diagnosis, planning, implementation, and evaluation. The RN fulfills a resident advocacy role by providing adequate information to make health decisions, coordinating total care, and working toward residents' highest practicable level of function.
    Ensures that resident care meets legal and professional standards and acts as a role model to other members of the healthcare team.

  • Essential Duties and Responsibilities:

    • Completes holistic, age-specific admission history, physical assessment, and nursing plan of care on all assigned residents.
    • Analyzes and incorporates clinical data such as lab and diagnostic results to develop individualized care plans.
    • Identifies and documents residents at risk for nutritional and functional status.
    • Identifies and reports signs or symptoms of neglect or abuse related to child, domestic, or elder abuse.
    • Sets priorities for and reviews the daily plan of care, collaborating with residents, families, and multiple healthcare disciplines.
    • Delegates to team members based on resident needs, continuity of care, staff, and knowledge.
    • Communicates with physicians to coordinate medical and nursing care plans.
    • Demonstrates competence in performing technical skills and managing urgent/emergent situations.
    • Administers medications and IVs per scope of practice and facility policies.
    • Documents nursing interventions, resident responses, and outcomes; evaluates and revises the care plan as needed.
    • Provides resident and family education, assesses learning needs, and documents discharge planning activities.
    • Maintains knowledge of CMS regulations and Medicare criteria to support compliance and appropriate care documentation.
    • Supervises, coaches, and evaluates performance of assigned CNA staff.
  • Additional Information:

    • Schedule: Rotating shifts starting with night shifts in October, transitioning to day shifts in November, with flexibility to work both days and nights.
    • Skills: Ability to read and write at a level necessary for documentation; physical ability to stand, walk, sit, push, pull, lift, carry, stoop, kneel, crouch, reach, file, type, xerox, talk, hear, and distinguish colors.
    • Physical Demands: Lifting objects over 100 lbs with frequent lifting/carrying up to 50 lbs.
    • Education: Graduation from an accredited School of Nursing (ASN) required.
    • Certifications & Licensure: Current RN license in the practicing state and BLS certification from the American Heart Association required.
