• Certifications and Education Requirements: BLS (Basic Life Support) Certification; Valid Drivers License; minimum of 2 years of experience


• Schedule: Wednesday and Friday shifts from 1745 to 0615, totaling 12 hours per shift, with a minimum of 36 hours per week


• Experience or 'Must Have' Skills: At least 2 years of hospice care experience involving end-of-life patient management


• Additional Information: Holiday requirement; comprehensive charting using EPIC electronic health record system; hospice unit setting; no float requirement; working on a hospice unit with a nurse-to-patient ratio of 1:6; involvement in start of care, recertifications, and daily visits; no trauma level designation; no home health or standby calls required; scrubs may be any color.
