Job Details:

- Required Qualifications: 1 year of skilled home health experience; OR and WA licensure; BCLS certification; valid license to practice as an OT in Oregon and Washington; experience with Oasis documentation; reliable transportation.
- Preferred Qualifications: Experience with National Patient Safety Goals and Core Measures; familiarity with billing procedures; proficiency in electronic medical records including EPIC and Rover; working with patients with diagnoses such as dementia, Parkinson's disease, ALS, multiple sclerosis, ortho post-op, or post-stroke.
- Additional Information: The traveler is expected to have their own transportation; the facility does not accommodate 4x10s or 3x12s shifts; occasional travel within a maximum radius of approximately 60 miles.
- Submission Requirements: List any time-off requests, provide three available interview times, confirm travelers phone number, indicate previous employment with the organization (travel, prn, perm), confirm reliable transportation, detail Oasis experience, and specify point expectations.
- References: Two references covering a minimum of 1 year of work experience within the last 3 years, including Charge RNs or Lead RNs if applicable.
- Additional notes: Travelers are expected to float between all facilities within their state; refusal to float may impact assignment eligibility.
