Position Summary:
Counsel and support Transitional Aged Youth (TAY) (THPP-NMD 18-21 and THP 21-25) in developing basic like skills, identifying community resources, and planning for their own self-sufficiency.
Pay Range: $24.55 - $35.08 per hour.
Requirements
Essential Duties:
* Acquires, maintains, teaches and demonstrates knowledge of Continuum of Care, Shared Core Practice Model, evidence-based practices, and trauma-informed care and maintains a professional environment that is consistent with Title 22, licensing regulations, and established agency policies.
* Assist Transitional Aged Youth to develop Transitional Independent Living Plan (TILP), Shared Living Agreement (SLA); and other required plans depending on program.
* Provide individual and group instruction in basic life skills.
* Provide individual and group instruction in job-readiness.
* Assist Transitional Aged Youth in identifying and accessing resources in their home community.
* Assist Transitional Aged Youth in obtaining documents such as birth certificates, ID cards, etc.
* Instruct and support young adult's access to public transportation.
* Assist Transitional Aged Youth n obtaining safe and appropriate housing.
* Assist and provide education to Transitional Aged Youth about higher education opportunities.
* Encourage and support Transitional Aged Youth transition to independence in accordance with their TILP.
* Meet with Transitional Aged Youth for face to face case management services or schedule regular contact meetings as outlined per agency and Community Care Licensing standards.
* Present psychosocial needs, strengths and concerns of Transitional Aged Youth during supervision.
* Report all serious incidents, allegations, grievances, and sensitive situations to Transitional Housing Program Director immediately.
Minimum Qualifications:
BA/BS in related field required, as is a minimum of two years working directly with Transitional Aged Youth. Working with special needs populations such as pregnant/parenting or Transitional Aged Youth with mental illness, behavioral problems, and/or system-involved strongly preferred.
Additional Requirements:
* Case Managers assigned to mono-lingual clients must be proficient in the Transitional Aged Youth primary language (if Spanish) or must be able to work effectively through interpreters.
* Must possess and maintain a valid, unrestricted California driver's license and provide safe, reliable transportation of young adults to necessary appointments and activities.
* Must be able to successfully complete and maintain required certifications and training including, but not limited to First Aid/CPR.
