Service Coordinator-Part-Time

Crimson Management is seeking a qualified candidate to fill the position of Service Coordinator. The Full-time Service Coordinator will service our Brook Point and Cottage Lane properties in Jackson, Georgia and Butler, Georgia. This position is designed to educate residents on available resources in community and assist them with obtaining the necessary services to age in place successfully, maintain housing stability, and live a healthy and independent life.

Full job description

· The Service Coordinator will work with residents individually as well as coordinate activities and programming that will promote mental and physical wellness and reduce isolation.

· The Service Coordinator connects residents to a multitude of social services which can include but not limited to home health care, case management, food assistance, transportation, legal advocacy, and local, state, and federal benefits.

· Acts as a liaison between community agencies, service providers, and residents and monitors the delivery of service to ensure they are appropriate, safe, timely, and satisfactory.

· Maintains a directory of community resources and makes it available to residents and staff.

· Works closely with property management staff to determine the needs of the residents (short-term or long-term care). This may include assistance with unit inspections to determine immediate needs.

· Raises awareness among local service providers of the community's needs and promote partnerships that help residents gain access to needed services.

· Weekly/Monthly communication with residents through newsletters and communication Boards

· Develop a sense of community among residents and their neighbors’ using organization and facilitation skills. This may include creating social/community-oriented programming, resident associations, welcoming committees, orientation packages, support groups, etc.

· Crisis Intervention demonstrates professionalism to handle challenging or stressful situations with confidence and composure.

· Maintains resident files and appropriate documentation.

REQUIREMENTS:

· Qualifications-Degree preferred, ideally in Social Work, Psychology, Counseling, or a related field. 1-3 years’ experience in social services or a related field, or an equivalent combination of education and experience.

· Valid Driver’s License

· Strong interpersonal, organizational, and communication skills

· Demonstrated ability to advocate, organize, direct /guide, problem-solve and provide results.

· Ability to act as a liaison between residents and management.

· Able to work independently or as a team member and support managers with special projects.

· The ability to interact and connect with a wide range of people, including residents and program providers, to monitor residents, their needs, and the services they're receiving.

· Crisis management as well as case management.

· Ability to maintain a high degree of confidentiality and professionalism.

Work Environment, Conditions & Physical Demands

· Work is performed indoors with occasional outdoor activities for wellness events.

· While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear, and taste or smell. The employee must occasionally lift or move up to 15 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
