Responsibilities
- Conduct initial and ongoing patient assessments, including OASIS documentation for admission, transfer, recertification, resumption of care, and discharge.
- Develop and implement individualized nursing care plans, ensuring compliance with physician orders, CHAP, Medicare, and state regulations.
- Perform skilled nursing procedures, including wound care, IV therapy, medication administration, and disease management education.
- Evaluate the psychosocial, environmental, and safety factors affecting patients and their families, making appropriate recommendations.
- Conduct comprehensive assessments of patients' health status and needs in a home environment.
- Develop and implement individualized care plans based on patient assessments and medical history.
- Administer medications, including injections, and monitor patients for side effects or adverse reactions.
- Perform venipuncture and handle insertion of intravenous needles and catheters, catheter care and other necessary medical procedures as required.
- Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
- Maintain accurate medical records and documentation of patient progress and interventions.
- Provide education to patients and families regarding health management, medication administration, and lifestyle modifications.
- Assist with outpatient procedures as needed, including those related to ambulatory surgery centers.
- Ensure compliance with safety protocols and infection control measures during home visits.
- Communicate and coordinate care plans with physicians, therapists, home health aides, and other interdisciplinary team members.
- Participate in case conferences and work with supervisors to optimize patient outcomes.
- Ensure timely and accurate documentation submission (progress notes, orders, patient updates)
Requirements
- Active Registered Nurse (RN) license in California (Required)
- CPR (Required)
- Home health experience is preferred
- Knowledge of OASIS, Medicare, Medicaid, CHAP, and HIPAA compliance
- Ability to lift, transfer, and assist patients safely
- Valid driver’s license and reliable transportation (Required)
- Familiarity with electronic medical record systems is a plus.
- Communication skills to effectively interact with patients, families, and healthcare teams.
- Ability to work independently while managing multiple patient cases effectively.
