Registered Nurse (RN) – Home Health (Per Diem / PRN)

What You’ll Do:
- Provide skilled nursing care to patients in their homes
- Perform admissions, routine visits, and patient assessments
- Develop and implement individualized care plans
- Educate patients and families on medications, treatments, and disease management
- Monitor patient progress and communicate changes with physicians and care team
- Complete timely and accurate documentation (EMR: DSL preferred)
- Collaborate with an interdisciplinary healthcare team
What We’re Looking For:
- Active California RN License
- Minimum 1 year of Home Health experience (within last 3 years)
- Strong clinical and communication skills
- Reliable transportation, valid driver’s license, and auto insurance
- Ability to work independently and manage time effectively
