Crescent Manor Nursing and Rehabilitation Center, located in scenic Bennington, VT, is actively seeking an experienced Coordinator for our Specialty HD Unit. The ideal candidate will have prior experience coordinating delivery of care in a long-term care setting.
The ideal candidate will be responsible for coordinating care on our HD Unit.
DUTIES:
Coordination of care for HD residents
Assist with compliance and ensure effective communication with all levels of nursing staff
Evaluate the outcome of practice and standards of care
Maintain a comprehensive knowledge of general nursing theory and practice
Responsible for the accurate and timely completion of medical records, care plans and documentation
Assist with interviewing, hiring and orientation of new nursing staff to the unit
Oversee daily staffing process for the unit
Maintain flexibility with work schedule to address unpredictable needs
REQUIREMENTS:
Current VT RN license required
Min. 3 yrs. exp. in Long-Term Care settings
2 years management experience preferred
Ability to work in a self-directed manner and in a team environment
Excellent Communication Skills required
Evidence of basic leadership skills and supervision
Flexible hours required
Crescent Manor Nursing and Rehabilitation Center is a charming, well-maintained, 90-bed rehabilitation and skilled nursing facility located in Bennington, VT. Crescent Manor houses a specialty Dementia Unit and a specialty Huntington’s Disease unit. Our staff is committed to ensuring the highest quality of life and care for all our residents, in a warm, nurturing environment.
The successful candidate will receive a $3500 sign on bonus along with a very generous compensation, benefits and PTO package.
