Responsibilities:
- Provide advocacy, linkage, and referral services on behalf of chronically mentally ill..
- Provide intake, screening, referral, and case management services.
- Assist in maintaining an active liaison with community agencies serving the “high-risk” population with the modified PACT program.
- Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF, and CREOKS rules and regulations.
- Attend and regularly present cases in the treatment team meetings.
- To read and be knowledgeable of all current CREOKS policies and procedures.
- Maintain productivity averages of direct service to clientele as set by CREOKS for full-time employment.
- Coordinate and manage the indigent medication/patient assistance program.
- Turn in billing and associated documentation as mandated by CREOKS policy.
- Responsible for understanding and following all of the administrative, clinical, and programmatic policies and procedures of the agency.
- Responsible for providing Psychiatric Social Rehabilitation Services, as assigned by Supervisor to agency Clients.
- Responsible for attending and presenting cases in the treatment team staffing conferences.
- Responsible for maintaining a full-time caseload as assigned by the Site Director and maintaining the daily productivity average as set by Administration.
- Responsible for billing documentation- to be completed on a daily basis, and clinical documentation to be completed daily, preferably, but no later than the beginning of the next work day.
- Responsible for necessary interaction with agency client families and/or other agencies involved with the client to ensure appropriate therapeutic treatment.
- Responsible for participating in mandatory trainings such as CAPE and CPR, etc.
- Responsible for implementing rehabilitative curriculum and developing support materials based on best practices and evidence-based treatment models.
- Responsible for teaching Rehabilitative skills that focus on, but are not limited to, skills necessary to perform activities of daily living and successful integration into community life. This service includes educational and supportive services regarding independent living skills, self-care, social skills regarding development, lifestyle changes, and recovery practices and principles.
- Treat all types of clients within the community based on the level of staff competency.
- All other duties as assigned.
- HS diploma or equivalent and 36 months of Behavioral Health experience or 60 college credit hours in any field and 12 months of BH experience or
- Bachelor's or master's degree in any field with 6 months of BH experience or
- Bachelor's or master's degree in the behavioral health field
- Case management certification and keep it active
- Basic knowledge and understanding of developmental stages
- Basic knowledge and understanding of treatment modalities.
- Excellent verbal, written communication skills.
- Ability to work as part of an interdisciplinary team.
- Core competencies in relation to individuals with co-occurring disorders.
- Must complete the SOC Wraparound 101 training.
