Case Manager-Mobile Primary Care

CREOKS Health Services
Tulsa, OK Contractor
POSTED ON 10/30/2026
AVAILABLE BEFORE 11/28/2026

CREOKS Health Services is establishing a mobile primary care clinic serving rural communities under the Oklahoma State Department of Health Rural Health Transformation Program (Rural Regional Reorientation). The Case Manager II performs the functions of a clinic-based case manager in the mobile setting: intake and eligibility screening, linkage and referral, follow-up scheduling, and connection of patients to CREOKS behavioral health services and to community resources in the communities served.

This position is funded through the RRR subaward. The role is based in a mobile clinical unit rather than a fixed clinic, and requires regular travel on a scheduled route among CREOKS locations.

Duties & Responsibilities:

  • Provide advocacy, linkage, and referral services on behalf of patients served by the mobile primary care clinic.
  • Provide intake, screening, referral, and case management services at each mobile clinic stop.
  • Conduct eligibility screening and assist patients with insurance enrollment, SoonerCare application, and sliding fee scale determination.
  • Coordinate follow-up appointments at the CREOKS West Tulsa, Bristow, or Okmulgee clinics, or other appropriate locations, and confirm patient attendance.
  • Assist patients in accessing community resources, including transportation, food, housing, and utility assistance, by referral, advocacy, and accompaniment as necessary.
  • Coordinate and manage indigent medication and patient assistance program enrollment.
  • Facilitate warm hand-offs between the mobile primary care clinic and CREOKS behavioral health and substance use services.
  • Maintain an active liaison with community agencies, tribal health programs, schools, and other partners in the communities served.
  • Assist with scheduling and community notification for mobile clinic stops.
  • Attend and regularly present cases in treatment team meetings.
  • Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF, and CREOKS rules and regulations.
  • Turn in billing and associated documentation as mandated by CREOKS policy; complete billing documentation daily and clinical documentation no later than the beginning of the next work day.
  • Maintain productivity averages of direct service to clientele as set by CREOKS for full time employment.
  • Assist with setup and breakdown of the mobile unit at each stop as needed.
  • Participate in mandatory trainings such as CAPE and CPR.
  • Read and be knowledgeable of all current CREOKS policies and procedures.
  • Responsible for understanding and following all administrative, clinical, and programmatic policies and procedures of the agency.
  • All other duties as assigned.

Qualifications / Competencies Required:

  • Minimum of Bachelor’s degree in a human services related field, or a Bachelor’s degree and at least two (2) years supervised experience in a treatment setting.
  • Current Oklahoma Case Management certification, or ability to obtain within [___] days of hire.
  • Valid Oklahoma driver’s license and an acceptable driving record.
  • Experience with SoonerCare, Health Insurance Marketplace, or sliding fee scale eligibility determination preferred.
  • Knowledge of community resources in Tulsa, Creek, Okmulgee, and Pittsburg counties preferred.
  • Basic understanding of social and cultural factors influencing human behavior.
  • Knowledge of various treatment modalities.
  • Capability to work as part of a small team with limited on-site support.
  • Core competencies in relation to individuals with co-occurring disorders.

Travel and Other Special Conditions:

  • This is a mobile position. Regular travel is required among the CREOKS West Tulsa, Bristow, and Okmulgee clinic locations, with occasional travel to the McAlester area.
  • Must be willing and able to work on a scheduled travel route and to work from a mobile clinical unit.
  • The mobile unit is towed as a truck-and-trailer combination with a gross combination weight rating above 26,000 pounds and a towed unit rated above 10,000 pounds. Under 49 CFR 383.91, that combination may lawfully be driven only by the holder of a Class A commercial driver’s license.
  • This position does not require a commercial driver’s license. The incumbent is not expected to tow, position or level the mobile unit. CREOKS assigns a qualified Class A driver for all movement and setup of the unit.
  • Candidates who hold a current Class A CDL, or who are willing to complete entry-level driver training and obtain one at CREOKS’ expense, are encouraged to indicate this. CREOKS may assign tow duty to a qualified clinical staff member and compensate it accordingly. This is optional and is not a condition of employment.
  • Work is performed in a mobile medical unit of limited size and at outdoor community sites. The position requires the ability to work in a confined space; to enter and exit the unit by steps or ramp; and to stand, walk, bend, reach, and lift in that environment.
  • Clinic days may begin and end at a CREOKS site other than the employee’s home location.
  • Travel reimbursed per CREOKS policy.
