Advanced Practice Registered Nurse (APRN) - Mobile Primary Care

CREOKS Health Services
Tulsa, OK Contractor
POSTED ON 10/30/2026
AVAILABLE BEFORE 11/28/2026

CREOKS Health Services is establishing a mobile primary care clinic serving rural communities under the Oklahoma State Department of Health Rural Health Transformation Program (Rural Regional Reorientation). The Advanced Practice Registered Nurse is the lead clinical provider on the mobile unit and delivers primary care services to patients across the lifespan at scheduled community stops.

This position is funded through the RRR subaward. The role is based in a mobile clinical unit rather than a fixed clinic, and requires regular travel on a scheduled route among CREOKS locations.

Responsibilities:

  • Serve as the primary care provider on the CREOKS mobile medical clinic, delivering direct primary care services to patients across the lifespan at scheduled community sites.
  • Perform comprehensive health assessments, including history, physical examination, and review of systems.
  • Diagnose and manage acute and chronic medical conditions within the APRN scope of practice.
  • Order, perform, and interpret screening and diagnostic tests appropriate to the mobile setting, including point-of-care testing.
  • Prescribe medications and therapeutic regimens consistent with Oklahoma APRN prescriptive authority and the collaborating physician agreement.
  • Provide preventive care, including immunizations, health screenings, and chronic disease management.
  • Identify behavioral health and substance use needs and coordinate warm hand-offs to CREOKS behavioral health services.
  • Supervise the Licensed Practical Nurse and Case Manager II assigned to the mobile unit, including day-to-day direction, scheduling, and performance input, and provide clinical direction within their respective scopes of practice.
  • Document all clinical encounters accurately and timely in the electronic health record, including when the unit is operating with intermittent connectivity.
  • Verify that medical equipment, medications, and vaccines carried on the unit are maintained within manufacturer and CDC storage and handling requirements, and report any deficiency or temperature excursion.
  • Participate in determining clinic schedules, stop locations, and patient flow for the mobile unit.
  • Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF, and CREOKS rules and regulations.
  • Read and be knowledgeable of all current CREOKS policies and procedures.
  • Maintain productivity averages of direct service to clientele as set by CREOKS for this job title.
  • Responsible for understanding and following all administrative, clinical, and programmatic policies and procedures of the agency.
  • All other duties as assigned.

Qualifications/Competencies Required:

  • Active, unrestricted license to practice in the State of Oklahoma as an Advanced Practice Registered Nurse.
  • Current national certification as a Family Nurse Practitioner (FNP) through the American Academy of Nurse Practitioners Certification Board or the American Nurses Credentialing Center is required. Certifications limited to the adult or adult-gerontology population do not meet the requirements of this position; the mobile clinic serves patients across the lifespan, including children. An Adult-Gerontology Primary Care Nurse Practitioner will be considered only with additional family practice training and certification covering pediatric populations.
  • Current Oklahoma prescriptive authority and DEA registration, practicing under a collaborating physician agreement with the Chief Medical Officer or designee, consistent with Oklahoma APRN rules.
  • Minimum of two (2) years of primary care or ambulatory care experience preferred.
  • Current BLS certification.
  • Valid Oklahoma driver’s license and an acceptable driving record.
  • Ability to practice independently with limited on-site support and to triage and manage urgent presentations in a non-hospital setting.
  • Comfort working independently in developing this program. This is a new service line, and the provider will help establish clinical workflows, standing orders, scheduling, and community-site routines as the mobile clinic begins operation.
  • Conceptual grasp of community-based and integrated primary and behavioral health care.
  • Core competencies in relation to individuals with co-occurring disorders.
  • Experience in rural health, FQHC, CCBHC, or mobile health settings preferred.

Travel and Other Special Conditions:

  • This is a mobile position. Regular travel is required among the CREOKS West Tulsa, Bristow, and Okmulgee clinic locations, with occasional travel to the McAlester area.
  • Must be willing and able to work on a scheduled travel route and to work from a mobile clinical unit.
  • The mobile unit is towed as a truck-and-trailer combination with a gross combination weight rating above 26,000 pounds and a towed unit rated above 10,000 pounds. Under 49 CFR 383.91, that combination may lawfully be driven only by the holder of a Class A commercial driver’s license.
  • This position does not require a commercial driver’s license. The incumbent is not expected to tow, position or level the mobile unit. CREOKS assigns a qualified Class A driver for all movement and setup of the unit.
  • Candidates who hold a current Class A CDL, or who are willing to complete entry-level driver training and obtain one at CREOKS’ expense, are encouraged to indicate this. CREOKS may assign tow duty to a qualified clinical staff member and compensate it accordingly. This is optional and is not a condition of employment.
  • Work is performed in a mobile medical unit of limited size and at outdoor community sites. The position requires the ability to work in a confined space; to enter and exit the unit by steps or ramp; and to stand, walk, bend, reach, and lift in that environment.
  • Clinic days may begin and end at a CREOKS site other than the employee’s home location.
  • Travel reimbursed per CREOKS policy.
