Job description
Formulate a plan of care for each of the patients under the directors care; following established standards of patient care and departmental policy; giving attention to physical, psychosocial, educational, and discharge needs of the patient and family. Perform direct care through utilization of the PHP process and in accordance with established policies and procedures. Maintains current files on each patient and makes recommendations to the facility staff as deemed necessary.
