Responsibilities:
- Conduct comprehensive patient assessments and Annual Wellness Visits
- Develop and implement patient care plans
- Administer medications and treatments
- Provide education to patients and their families on health management
- Collaborate with physicians and other healthcare professionals
- Utilize E-Clinical Works or similar electronic health record systems
Skills:
The ideal candidate should possess the following skills:
-Ability to see 18-20 Patients in clinic environment
- Experience with telehealth services
- Knowledge of durable medical equipment (DME)
- Competence in utilization management practices
- Skill in medication administration
- Expertise in dementia care
- Proficiency in medical terminology
