Creative Minds is seeking a Case Manager. If you are a natural leader and are work positively with others, this is a position for you.

Creative Minds ILS/SLS services adults with developmental disabilities. Case Managers will support with the coordination of caregivers, and will work alongside other Case Managers, Program Supervisor, and Program Director. Case Managers will coordinate, and schedule client support and services, mentor and manage a team of direct care professionals, provide quality assurance, complete monthly, quarterly, and annual reports, and intake assessments.

Under the direction of the ILS / SLS Program Supervisor and Program Director; core duties and responsibilities include the following. Other duties may be assigned.

  • Screens clients for intake appointment and/or referrals either in person, on the phone, or on emergency basis.
  • Provide case management services, using both group and individual ILS/SLS model.
  • Coordinate and arrange services hours, implement schedules for staff and recipient coverage. Ensure service hours are implemented within the authorized level of service.
  • Provide crisis intervention services to manage immediate concerns/risk; provide initial support/assistance; and help develop and enhance the working relationship between the client and the Center.
  • Participate as a member of a multi-disciplinary team. To attend all scheduled staff meetings to assist in assessing the client’s needs, strengths, resources, and provide feedback regarding treatment planning.
  • Network effectively and provide co-location orientations with DPSS, GAIN, Job Club, and other community resources as needed.
  • Complete all chart documentation, reports, coordinated services plans, need assessments, outcome and measurements, and other received paperwork within the time frame required.
  • Participate in home visits to clients when necessary and safe to do so.
  • Provide unmatched client customer service, direct service to service recipients when required, in accordance with the recipient’s service plan through the agency
  • On-Call availability in accordance with current policy in place
  • Gather relevant case-specific information, data and documents for client files, at-home binders, and updated reports to send to CSC.
  • Coordinate and track cases with Program Manager, and provide support when needed
  • Coordinate with other professionals such as trust attorneys and other vendors that may be relevant for the client

Education and/or experience requirement:

High school diploma or general education degree (GED) with five years of related experience and/or training; or Associates degree with three years of related experience and/or training, or Bachelors degree with one year of related experience and/or training.

Preferred candidate characteristics

  • Knowledge/experience with the Regional Center and DDS protocol
  • Field experience
  • Fingerprint/background clearance required - no felonies or criminal record
  • Acceptable DMV record
  • Valid CA Driver's License
  • Working and reliable vehicle, auto insurance
  • Health screenings clearance
  • Eligible to work in U.S.

Pay and benefits

Rate of pay can range from $24.00 to $26.00 per hour based on experience and education, may increase within pay range. Workdays are Monday through Friday, 8 hours a day. Travel

Creative Minds provide services in the San Fernando Valley, Santa Clarita, and Antelope Valley areas. It is necessary to travel throughout these areas as part of the responsibilities associated with the position. A valid California Driver's License, Car Insurance, and a reliable vehicle are requirements. As travel through the Valley is a necessary part of the position, a mileage allowance will be provided. Services will be provided in-person.

The employee must regularly lift and /or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.

While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
