Regional Nurse

$5,000 Sign-On Bonus

Creative Community Living Services is an agency providing in-home support for adults with disabilities and the frail/elderly in a variety of settings. We are seeking a compassionate and skilled Regional Nurse to join our team in the Greater Milwaukee area, including Waukesha, Bayside, New Berlin and Greenfield. This role provides both administrative oversight and hands-on care to ensure the highest quality of services for our clients.

Key Responsibilities:

• Deliver and monitor nursing care in line with physician orders, state regulations, and nursing standards.
• Conduct health assessments, medication reviews, and audits.
• Monitor MARS, change of condition and other medical needs. 
• Delegate, educate, mentor, and support direct care staff in medically oriented tasks.
• Collaborate with physicians, client teams, and ancillary agencies to coordinate services.
• Participate in pre-admission assessments, hospital discharge follow-ups, and client evaluations.
• Ensure compliance with CBRF and AFH Regulations.

Qualifications:

• Current Wisconsin RN license in good standing.
• Knowledge of applicable DHS regulations and nursing standards.
• Experience with individuals with developmental disabilities in CBRF and AFH settings
• Strong communication, organizational, and leadership skills.

Job Types: Full-time, Permanent

Pay: From $40.00 per hour
