Reentry Coordinator (Grant Funded, Time Limited)

This is a temporary, time-limited, grant funded position through June 30, 2026.
Performs professional and administrative work in connection with the County’s reentry program aimed at reducing recidivism and promoting successful community reintegration for justice-involved individuals. Conducts intake assessments and develops individualized reentry plans for Craven County inmates.
Coordinates service referrals for assistance with housing, employment, medical, behavioral health, substance use treatment, and social services.
Facilitates warm handoffs and closed-loop referrals to community partners.
Maintains regular communication with clients, service providers, and correctional staff.
Tracks participant progress and maintains accurate documentation for grant reporting.
Provides crisis intervention and support to inmates, as needed
Participates in case review meetings and multidisciplinary team coordination.
Monitors outcomes and assists with program evaluation activities.
Ensures services are trauma-informed, culturally responsive, and aligned with best practices.
Ensures compliance with COSSUP grant requirements. 
Establishes and maintains cooperative relationships with representatives of community partners and coordinates activities with other agencies and private organizations to enhance program activities and service availability. Graduation from an accredited college with an associate’s degree in human services or criminal justice and two or more years of experience in case management, reentry services, or related work or an equivalent combination of related education, training or experience. A bachelor’s degree in social work, criminal justice, public health, or related field is preferred. Must be a North Carolina Certified Peer Support Specialist.

Special Requirements
Possession of a valid North Carolina driver’s license and reliable transportation.
Must be able to obtain background check clearance required for working in detention facilities.

Knowledge, Skills, and Abilities
Comprehensive knowledge of community resources, criminal justice system, alternatives to incarceration, correctional treatment practices, local, state and federal laws, guidelines, and programs. Basic working knowledge of inmate sentencing, pretrial conditions, and release.
Thorough knowledge of the principles and practices of case management and interviewing.
Thorough knowledge of community resources and services.
General knowledge of mental health and substance use disorders and available treatment and recovery resources. 
Ability to use Microsoft Office 365 to include Word, Excel, PowerPoint, Outlook and Teams.
Ability to exercise sound judgement in routine and emergency situations.
Ability to work in a hazardous and confining working environment.
Ability to prepare and maintain accurate, thorough, and organized records.
Ability to communicate effectively both orally and in writing.
Ability to work independently and as part of a multidisciplinary team. 
Ability to establish and maintain effective working relationships with a wide variety of community partners, court staff, and coworkers.  Ability to deal tactfully and firmly with inmates and the public.
Ability to multitask, prioritize duties, and to manage multiple cases.  
Ability to operate office equipment, computers and applicable software applications.  
Ability to safely operate a motor vehicle. 
Ability to attend work regularly; and ability to maintain confidentiality.Participation in disaster preparedness and response operations should be expected in the event of an emergency. Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency.

Physical Requirements
Must be able to perform light work exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. Vocal communication is required for expressing or exchanging ideas by means of the spoken word; hearing is required to perceive information at normal spoken word levels; visual acuity is required for compiling and computing data and information, for extensive reading and research, and proofing and inspecting finished written material as well as preparing and analyzing written or computer data, determining the accuracy and thoroughness of work, and observing general surroundings and activities; the worker is not subject to adverse environmental conditions. Must be able to physically perform the basic life functions of stooping, standing, walking, grasping, fingering, talking, and hearing.
