Support Coordinator

Qualifications
- Bachelor’s degree min. required
- Must be able to pass a Level 2 background screening
- Minimum of 2 years of experience working with individuals with behavioral health needs and/or developmental disabilities
Position Details
- Initial training will be completed at our office located in Haines City
- Client home visits are required based on individual living settings, with most visits occurring no more than once per month
- Flexible, community-based work environment
- Make your schedule as needed
- consumer locations are based in Polk, Highlands, Hardee counties.
Responsibilities
- Maintain accurate and up-to-date progress notes and central files within the iConnect system
- Coordinate and monitor services provided by multiple care providers and agencies
- Assist individuals in accessing community resources, programs, and support networks
- Advocate for the rights, goals, and needs of the individuals served
- Build positive relationships with clients, families, and community partners
