Care Coordinator
Onsite in Lake Mary, FL | Monday–Friday | 8:00 AM–5:00 PM
HealthCare Support is seeking Care Coordinators to join a rapidly expanding healthcare company. This is a patient-focused healthcare role supporting individuals receiving in-home primary care, chronic care management, remote patient monitoring, palliative care, and hospice services.
Care Coordinators are the central connection between patients, caregivers, providers, hospitals, insurance organizations, and other healthcare resources. You’ll help patients resolve barriers to care, coordinate follow-up, manage electronic medical record workflows, and ensure important requests are routed to the appropriate clinical team.
Compensation
- $18.00–$19.00 per hour
- After 90 days, eligible employees may participate in a pay incentive program, paid biweekly based on KPI achievement
- Employees who earn a CCMA or MA certification after six months receive a permanent $1.00 per hour increase
Schedule
- Monday–Friday, 8:00 AM–5:00 PM
- Flexible option: 8:30 AM–5:30 PM

Daily Responsibilities
- Handle approximately 20–30 inbound patient calls each day
- Assist patients and caregivers with medication concerns, equipment requests, medical records, and care transitions after hospitalization
- Coordinate referrals, laboratory orders and follow-up, medication refill requests, and durable medical equipment
- Complete insurance verifications and approval-related tasks
- Coordinate provider schedules, visits, and follow-up care
Qualifications
- High school diploma or GED equivalent
- Experience in a patient-facing or member-facing healthcare position, such as medical assistant, CNA, medical front office, care coordination, patient outreach, healthcare call center, or a related role
Growth Opportunities
- Advancement opportunities after 8-12 months, including greater responsibility, more complex providers, and senior-level opportunities.
- High performers may earn 1-2 remote days per week.
