Position Summary
The Peer Support Case Manager provides direct support, guidance, and mentorship to shelter clients, leveraging their own lived experience to foster trust, connection, and hope. This role focuses on building relationships with clients, assisting them in navigating services, and supporting their journey toward stable housing and self-sufficiency. The Peer Support Case Manager works collaboratively with shelter staff to create a welcoming and supportive environment that prioritizes client-centered, trauma-informed care.

Key Responsibilities

Client Support and Advocacy:

Build trusting relationships with clients by sharing personal experiences of overcoming challenges, as appropriate, to inspire hope and resilience.

Provide emotional support, encouragement, and motivation to clients as they work toward achieving their housing and personal goals.

Assist clients in identifying and accessing resources, including housing, employment, health care, and social services.

Empower clients to develop self-advocacy skills and confidence in navigating systems and overcoming barriers.

Accompany clients to appointments or meetings as needed, offering support and guidance.

Program Engagement:

Facilitate peer-led support groups, workshops, or activities designed to enhance life skills, coping strategies, and community building, employment and housing.

Collaborate with shelter staff to ensure clients are connected to appropriate case management and housing resources.

Participate in weekly case conferences to provide insights and updates on clients’ progress.

Promote a safe, inclusive, and supportive environment by addressing conflicts or concerns with empathy and professionalism.

Stay informed about community resources, housing programs, and best practices in peer support.

Documentation and Reporting:

Maintain accurate and timely documentation of interactions, progress, and challenges faced by clients.

Provide input for reports and program evaluations to support continuous improvement of shelter services.

Data input and inputting daily case notes

Housing planning

Other Duties:

Perform additional tasks as assigned by the supervisor to support shelter operations and ensure the well-being of residents.

Supervisory Responsibilities

This job has no supervisory responsibilities.

Required Knowledge, Skills, and Abilities:

Knowledge of supportive housing programs, affordable housing resources, and community services.

Understanding of homelessness, mental health issues, substance use, and trauma-informed care.

Strong organizational, communication, data entry, and advocacy skills.

Ability to work independently and manage a high caseload in a fast-paced environment.

Sensitivity and understanding when working with vulnerable and diverse populations.

Commitment to equity and inclusivity.

Qualifications

High school diploma or GED required.

Lived experience preferred.

Bilingual (English/Spanish) preferred.

Certification as a Peer Recovery Specialist preferred.

Certificates, Licenses, Registrations

Valid Driver's License preferred, CPR Certification preferred

Job Type: Part-time

Pay: $19.00 - $21.00 per hour
