The Home Health and Hospice Technician is responsible for providing personal care and supplementary service as designated on the plan of treatment. The position works under the supervision of the registered nurse, LPN, or other professional for an assigned unit. Demonstrates excellent customer service performance in that his/her attitude and actions are consistent with the standards contained in the Vision, Mission and Values of Covenant HealthCare and the commitment to Extraordinary Care for Every Generation.
Responsibilities
Demonstrates excellent customer service
Has the emotional maturity to work with dying patients and their family.
Works at the patient’s home. The patient’s home may be a private residence, assisted living facility, a skilled nursing facility, or a hospital.
Contributes to organization success targets for patient satisfaction and net operating margin.
Provides assigned personal care and simple treatments to the clients to assist in the continuity of care.
Follows the plan of care established by the Case Manager.
Supports and demonstrates assigned care measures to the family members.
Assists the client towards independent living when possible.
Reports requests of clients and families to the supervising registered professional nurse.
Reports changes in physical status to the supervising registered professional nurse.
Develops and maintains empathetic communications with the client and family.
Completes documentation requirements.
Participates in continuing education/in-services to maintain fourteen hours per year.
Other duties as assigned.
Qualifications
EDUCATION/EXPERIENCE REQUIREMENTS
High school diploma or G.E.D. required.
Current CNA certification in the State of Michigan required or obtain certification within 3 months of hire or transfer required.
BLS required.
One (1) year experience preferred as a CNA, HHT, HCT or related position
KNOWLEDGE/SKILLS/ABILITIES
Ability to deal effectively with the demands of the job. Patients with limited life expectancy. Ability to recognize limitations and scope of own role, and seek supervision when indicated.
Ability to read, write, and follow directions. Capable of meeting Competency Evaluation Test (OBRA-8Ab7).
Ability to perform all aspects of the role as HHT. Able to lift, turn, and transfer patients without restrictions.
Knowledge of Microsoft Excel, Word and Outlook.
Valid State of Michigan driver’s license, reliable transportation and proof of auto insurance required
WORKING CONDITIONS/PHYSICAL DEMANDS
Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
Frequent standing, walking, lifting, carrying, pushing, pulling, balancing, handling, talking, and hearing.
Frequent near and far vision, color and field of vision, and depth perception.
Frequent lifting up to 100 lbs.
Occasional sitting, stooping, kneeling, crouching, squatting, twisting, reaching, tasting and smelling.
