Join a PACE/LIFE senior care program where you’ll provide ongoing primary care to older adults while working closely with an interdisciplinary clinical team. You’ll get to know your patients, help manage their care over time, and have a team to support you.

What You’ll Do

• Provide primary care, assessments, and treatment for older adults
• Develop and update individualized care plans
• Work closely with physicians, nurses, therapists, social workers, and other members of the care team
• Order and review labs, diagnostic testing, and other clinical services
• Respond to changes in participant health and coordinate follow-up care
• Educate participants and families on health conditions and care plans
• Maintain timely and accurate clinical documentation

What You’ll Bring

• Active Pennsylvania Nurse Practitioner license
• Experience working with older adults is preferred, but training is available
• Current CPR and First Aid certification
• Valid driver’s license and ability to travel locally for participant visits and training
• Ability to work independently while collaborating closely with a clinical team
