Position Description: Provides support and services designed to ensure effective, quality mental health services and treatment for clients of the Health Services Agency. Serves as an advocate for clients in various daily living situations.
Case Resource Specialist I/II:
Assist adult/children’s outpatient team with service authorization determinations.
Coordinates with community services and businesses to provide clients maximum assistance.
Case Resource Specialist I:
Meets with clients and consults with service providers on a regular basis to ensure the proper implementation of case management plans and to monitor case progress.
Refers clients to and/or coordinates care plans with other private or public mental health and medical service providers and support programs as appropriate.
Case Resource Specialist II:
Assesses and evaluates families and individuals, including but not limited to mental status, economic need, vocational potential, physical health and needs for socialization, placement, assistance with living arrangements, transportation and appropriate treatment.
Develops service plans for individuals and families referred to Case Management Services.
Maintains contact with families and individuals as often as necessary to assist in the implementation of service plans.
Bilingual Candidates: Translates, interprets and otherwise communicates with members of the Spanish speaking community.
Case Resource Specialist I/II: Graduation from an accredited college or university with an Associate’s Degree in a health or social services related field or graduation from an accredited college or university with a Bachelor’s Degree in a health or social services related field.
Case Resource Specialist I:
- One year experience working with mental health clients or other special needs populations preferably in a County Mental Health system;
Case Resource Specialist II:
- Six years of experience working with mental health clients or other special needs populations preferably in a County Mental Health system
OR
- A Bachelor’s Degree - four years of experience working with mental health clients or other special needs populations preferably in a County Mental Health system. Up to 2 years of graduate professional education may be substituted for the experience requirement on a year-for-year basis.
License & Special Requirements:
- Requires a valid California driver’s license
- Must possess or be willing to obtain a National Provider Identifier (NPI).
- Case Resource Specialist II: Must be eligible for certification as a Mental Health Rehabilitation Specialist per Title 9, Division I, Chapter 3, Article 8, Section 630 of the California Code of Regulations.
Bilingual Candidates: Must pass a Cooperative Personnel Services (CPS) written and/or oral bilingual proficiency test (or other appropriate test selected by the County) to qualify.
The department is recruiting for one (1) current vacancy.
In addition, Tehama County is establishing an eligibility list to fill future vacancies for a Case Resource Specialist I/II or Case Resource Specialist I/II - Bilingual, as well as subsequent vacancies which occur during the life of the eligibility list.
