What are the responsibilities and job description for the Social Worker/Case Manager Adult CCS position at COUNTY OF BROWN?
JOB REQUIREMENTS: Provides appropriate mental health services, including
intake, assessment, case management, and direct services to consumers
who have mental health and substance use issues (when co-occurring),
including engaging and providing/facilitating provision of these
services at community sites. Essential Duties Screens and evaluates
individuals to determine appropriate services based on individual
consumer needs. Engages consumers in recovery based process. Develops
and implements service plans that provide for appropriate mental
health/co-occurring services to consumers and their families, where
necessary. Provides primary case management for assigned consumers,
including assessment, treatment planning, monitoring, and evaluation.
Provides appropriate mental health services to a caseload with severe
and persistent mental illness and alcohol and drug abuse issues.
Provides direct community-based services when appropriate to Community
Treatment Program consumers to include, but not limited to: - Assists in
learning and maintaining adequate activities of daily living skills. -
Support for consumers and families in crisis or stress situations. -
Assists consumers who have difficulty with self-management of
medication. - Assists consumers who have difficulty in maintaining
regularly scheduled appointments. - Assists in finding and maintaining
appropriate housing arrangements. - Serves as liaison agent to other
support systems in the community. - Assists through group and individual
contacts to improve consumers\' problem-solving abilities. - Transports
consumers in relation to the above services when necessary. Provides
crisis intervention/service facilitation as necessary. Maintains the
confidential nature of consumer and business information. Participates
in admission and discharge planning with staff from the Brown County
Community Treatment Center Nicolet Psychiatric Hospital and other
inpatient units. Complies with all local, state, and federal laws
governing consumers and the rendering of services. Provides community
education to the public and various agencies in the area of mental
health and co-occurring issues. Provides consultation to staff of other
human service departments and agencies. Monitors contracted services.
Attends in-service meetings and outside agency training sessions.
Completes and maintains necessary documentation according to applicable
Requirements, Codes And Policies. Performs Related Functions As

casework principles and methods, social systems, family dynamics, group
and individual behavior. Knowledge of dynamics involved in community and
institutional care for persons with mental illness and co-occurring
disorders. Knowledge of professional interviewing principles and
techniques. Knowledge of community resources and utilization. Knowledge
of Wisconsin State Statutes Chapter 51 and related court procedures.
Ability to maintain records and reports accurately. Ability to assess
problem situations, specifically in the area of mental health, and
determine the need for services. Ability to use authority
constructively. Ability to plan and organize work and to make effective
use of time. Ability to communicate effectively both orally and in
