What are the responsibilities and job description for the Practical Cents Case Manager position at County of Adams?
Performs intermediate skilled human service work by providing person-centered support services to trainees at Practical Cents. Provides guidance and training to trainees with the goal of improving personal and professional interactions, learning work skills, and supporting individualized service plans and pre-vocational plans. This position requires the ability to work a minimum of one Saturday per month.Work is performed under limited supervision of the Practical Cents Store Manager.

Utilizes a trauma-informed approach to provide person-centered support services to clients in behavioral health and long-term support programs.
Performs person-centered assessments for pre-vocational plans and develops goals for the individual.
Coordinates delivery of services described in the client's recovery plan/individualized service plan, collects data and prepares documentation.
Participates in monthly, semi-annual, and annual meetings with stakeholders as necessary or required.
Maintains client records in accordance with regulatory guidelines and provides accurate and timely case management documentation.
Assists in program and operation improvement solutions.
Serves as point of contact for the Health and Human Services Department relative to assigned external agencies, programs, or services and represents the department at professional meetings and community events as required.
Consults and receives supervision from other professional staff within the agency.
Completes duties and responsibilities in all areas of the store with and/or without trainees including receiving, workroom, storefront and other off site work locations.
Performs other related functions and duties as assigned or required.

Bachelor's degree in social work, sociology, human services, or related field and moderate experience working with various client populations, or equivalent combination of education and experience. Valid driver’s license in the State of Wisconsin.

Requires the patience to work with individuals with intellectual and physical disabilities and a variety of interfering behaviors. Requires the ability to work with minimal supervision in a fast-paced and stressful environment focusing on client treatment and habilitation; must be able to monitor up to 20 clients (including whereabouts, quality of production, and behaviors).
