POSITION TITLE: Case Manager II (Mental Health Court)
Minimum Qualifications
Case manager experience preferred. Bachelor's Degree in Human Services field from an accredited college/university. Case manager II certification or eligible to obtain level II certification. .
The Mental Health Court Case Manager Is Responsible For
PHYSICAL REQUIREMENTS: Must be able to move from location to location easily, bending, stooping, lifting 25 pounds, communicate clearly, prepare written communications, present information to other employees, process complex information within established procedures/programs, enter data to various programs. Must be able to climb stairs and access non-handicapped equipped residences/facilities, may be exposed to diseases, illness, or infection, ability to use basic office equipment such as: telephone, calculator, computer, etc.

HIPAA: This position is permitted to locate information necessary to follow through with client care or provide authorization for release of information outside agency. Information should be requested from Medical Records staff initially or WITH authorization from administration, retrieve necessary information without supervision. Level 2 access.
REPORTS TO: Program Director
EMPLOYMENT STATUS: Exempt
SUPERVISES: None
HOURS: Full time. May be required as needed to ensure quality standards are met in service delivery.
This position has a weekly productivity requirement of 25 billable services each week. Documentation must be entered into the EHR within 48 hours of providing the service per agency standards.

The Mental Health Court Case Manager Is Responsible For
- Coordinating all services provided to mental health court clients.
- Participates in mental health court staffing and clinical staffing to determine the ongoing needs of the client.
- Attends weekly court proceedings to advocate and support clients.
- Assists with clients’ case management needs, including but not limited to, housing, employment, inpatient treatment, and other basic needs.
- Facilitates individual and group services such as wellness and rehab.
- Meets with clients at the office, jail, and in the community to address immediate needs and provide referrals.
- Monitors clinical pathways and service delivery by maintaining routine contact with clients and other service providers involved with clients and helps identify other resources that will help the clients’ recovery process, including informal support and network building to help clients increase their support system outside of the mental health system.
- This staff also ensures proper and timely maintenance of case management records with appropriate reviews and updates of service plans.
- Case manager experience preferred.
- Bachelor's Degree in Human Services field from an accredited college/university.
- Case manager II certification or eligible to obtain level II certification.
- Work environment is office and community based. Must provide own transportation.
- Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organize and present facts effectively.
- Ability to establish working relationships with clinical, administrative, and support staff in order to develop professionalism and coordination within the team.
- Professional presentation of oneself at all times with the ability to establish and maintain effective, cooperative working relationship with fellow employees, clients, school personnel, and the public while maintaining objectivity and a positive, collaborative attitude.
- Must possess good working knowledge of community social service resources, basic crisis management skills, behavioral health resources, ability to provide trauma-informed care and a commitment to the Sanctuary Model components.
- Ability to communicate clearly and in an emotionally intelligent manner with clear, polite, and respectful communication with staff, youth served, administration, public school personnel, and the community- in person, through written correspondence and on the telephone.
- Must always demonstrate professional and ethical behavior as well as the ability to maintain strictest confidentiality guidelines for interactions regarding client information.
- Ability to work well under pressure meeting deadlines with efficiency and consistency while maintaining a balance between good physical and mental health and the job requirements.
- Such specialized training and/or certification in mental health services or related field as may be required to provide adjunctive services, i.e. Child/Adult Case
- Management Certification; BHRS, if requested.
