Cottage Hospital is seeking a compassionate and motivated Registered Nurse to join our team as a Per Diem Cardiac Rehab RN. This role offers a flexible, as-needed schedule and the opportunity to play a key role in supporting patients through their cardiac recovery and overall wellness journey.
In this position, you will conduct initial patient assessments, develop and implement individualized care plans, and monitor patient progress throughout their rehabilitation. You’ll work closely with providers and the broader healthcare team to ensure safe, effective, and patient-centered care, while also serving as a positive role model for healthy lifestyle practices.
Key Responsibilities
- Perform initial patient assessments and develop individualized care plans based on clinical evaluation, provider orders, and established protocols
- Monitor patient response during rehabilitation sessions and adjust care as appropriate
- Initiate and respond to emergency protocols as needed
- Accurately document and communicate patient status and progress
- Collaborate with providers and interdisciplinary team members to support coordinated care
- Participate in staff education, program development, and ongoing program evaluation
- Support patient education and promote healthy lifestyle habits
