Cottage Hospital is seeking a detail-oriented and collaborative Per Diem Pharmacist to support our pharmacy team. This flexible position offers the opportunity to contribute to both clinical care and operational excellence while working under the guidance of the Pharmacy Director.
In this role, you will provide safe, effective pharmaceutical care while ensuring compliance with all federal, state, and local regulations. You’ll play a key part in supporting patient outcomes through clinical collaboration, medication management, and efficient pharmacy operations.
Key Responsibilities:
Clinical Pharmacy:
- Participate in multidisciplinary rounds to support optimal medication therapy
- Provide consults on pharmacokinetics, antibiotic stewardship, anticoagulation, and drug information
- Document medication interventions and pharmacy care notes in the EMR
Pharmacy Operations:
- Verify medication orders and support accurate charge capture using pharmacy systems
- Prepare and verify unit dose medications and non-hazardous sterile compounds
- Maintain compliance with USP 797/800 standards
Collaboration & Support:
- Work closely with medical and nursing staff on medication therapy, order clarifications, and safety procedures
- Supervise and support pharmacy technicians in daily operations
- Participate in meetings, in-services, and departmental initiatives
Inventory & Compliance:
- Assist with inventory management, ordering, and maintaining appropriate stock levels
- Ensure accurate recordkeeping for all medications, including controlled substances
- Support monthly inspections of medication storage areas
Additional Responsibilities:
- Maintain pharmacy cleanliness, handle communications, and support general workflow needs
- Uphold all infection control and safety policies
Qualifications:
- Licensed Pharmacist in the State of New Hampshire (or eligible)
- Strong clinical knowledge and attention to detail
- Ability to work independently while collaborating with a multidisciplinary team
