Job Summary
The Care Coordinator RN works in close collaboration with patients, families, primary care providers, specialists, and community partners to ensure timely access to care and continuity across the healthcare continuum. This role focuses on supporting chronically ill and high-risk patients, improving preventive care utilization, and reducing emergency department visits and hospital readmissions.
In addition to care coordination, this position plays a key role in clinical triage, medication refills, and medication prior authorizations. The Care Coordinator RN uses clinical judgment, assessment skills, and critical thinking to guide patients toward appropriate levels of care and support safe symptom management.
Key Duties & Responsibilities
Care Coordination & Patient Support
- Partner with high-risk and chronically ill patients and their caregivers to support individualized care plans
- Coordinate care across clinics, hospital departments, specialty providers, and community resources
- Support care transitions following hospital admissions, discharges, and emergency department visits
Conduct comprehensive preventive services, including:
Medicare Annual Wellness Visits
- Transitional Care Management (TCM)
- Chronic Care Management (CCM)
- Advance Care Planning
- Promote patient self-management, education, and behavior-change strategies
- Monitor patients with multiple co-morbidities or high risk for readmission and intervene proactively
Triage & Clinical Coordination
- Assist Clinic RNs with telephone triage, conducting initial patient assessments and determining urgency
- Use clinical judgment to guide patients toward appropriate levels of care (home care, clinic visit, urgent or emergent care)
- Assist with medication prior authorizations, denials, and appeals
- Support providers by tracking, monitoring, and completing medication refill requests
Team Collaboration & Documentation
- Promote clear communication across the care team to ensure alignment on patient care plans
- Maintain accurate and timely documentation in the electronic medical record
- Perform other duties as assigned to support clinic operations and patient care
Experience
Preferred
- Previous practice as a RN in an outpatient setting (preferred)
Education
Required
- Associates or better in Nursing or related field
Licenses & Certifications
Required
- Basic Life Support
- RN License
